VDP Terms and Conditions – 2024
The Verber Dental Plan is a dental savings plan and is not a qualified health plan under the Affordable Care Act. The membership plan is a direct agreement between plan members and their dentist, eliminating the cost and hassle of a middleman.
The Verber Dental Plan is not dental insurance and cannot be utilized in conjunction with any dental insurance or other 3rd Party Payer.
The membership fee for the first household individual is $265, regardless of the enrollee’s age.
Your membership will auto-renew and the credit card on file will be billed on the anniversary date of your registration.
A full refund of the membership fee will be granted within 30 days of registering, as long as no services were utilized.
Rates are subject to change on an annual basis.
To be eligible for the Loyalty Plan discount of 25%, you must remain in good membership standing for 5 consecutive years with no lapse in membership coverage.
Discounts do not apply to completed services or treatments that are in progress.
The discount applies to work within the scope of our Verber Dental Plan participating provider’s sole discretion.
If you are referred to an outside specialist, they will not offer this discount. This plan does not apply to any other promotional or discounted services.
This discount applies to dental services only and excludes dental products purchased in our office.
Discounted payments are due at the time of service. Services not paid in full on the day of treatment are ineligible for a discount and will be billed at our regular rate.
If utilizing CareCredit, the discount for services will be reduced by 5%. (i.e. If your discount is 15%, using CareCredit will lower the discount to 10% off services)
Membership participation is required through the completion of any extended treatment plans to receive Verber Dental Plan discounts.
We reserve the right to terminate membership at our discretion for nonpayment of membership or dental fees, missed appointments, and/or lack of a healthy doctor-patient relationship. If we do so, we will give you a prorated refund based on the number of months remaining on your plan and the services used.
We reserve the right to refuse enrollment to anyone for any reason not prohibited by law.
We respect your privacy. Information collected on this website will not be sold to third parties or utilized for any outside marketing purposes.
There is no guarantee of scheduling priority and plan utilization if appointments are canceled.
This plan cannot be assigned, sold, or transferred by the patient.

Verber Dental Plan Fees & Included Services (Schedule A) The Verber Dental Plans provide various plans, discounts, and inclusive benefits.
Individual & Family Plan: Plan fee is $265 for the first person registered in the household, regardless of age. Each additional adult member fee is $215, and children under the age of 13 are $165. Preventive Services listed below are inclusive with the annual plan. An additional discount of 15% on all other dental services will be applied with payment at the time of service.
Group Plan: A group consists of five participating different households registered. If an existing group cannot maintain a status of five households, it cannot re-register the next consecutive year for the group discount. Plan fee is $265 for the first person registered in the household, regardless of age. Each additional adult member fee is $215, and children under the age of 14 are $165. Preventative Services listed below are inclusive with the annual plan. An additional discount of 20% on all other dental services will be applied with payment at time of service for all members of the group.
Loyalty Plan: After maintaining five consecutive years of plan membership, the member will be eligible for the 25% Loyalty discount on all services. Plan fee is $245 for the first person registered in the household, regardless of age. Each additional adult member fee is $215, and children under the age of 14 are $165. Preventative Services listed below are inclusive with the annual plan. An additional discount of 25% on all other dental services will be applied with payment at time of service. If a lapse in membership occurs, the loyalty discount will be reduced to the Individual or Group plan discounts.

Dental Cleanings: A total of two times per contract period (12 months beginning at your sign up date) of any dental code listed below: Adult Prophy (D1110) Child Prophy (D1120) Periodontal Maintenance (D4910)
Examinations: A total of two times per contract period (12 months beginning at your sign up date) of any dental code listed below: Comprehensive Exam (D0150) Periodic Exam (D0120) Comprehensive Periodontal Exam (D0180)
Radiographs: 1 set of bitewing radiographs per year & 1 full set of X-Rays every 3 years. 4 Bitewing Adult (D0274) 2 Bitewing Child (D0272)) Panoramic Radiograph (D0330) FMX (D0210)
Fluoride: 1 fluoride treatment every 6 months with dental cleaning appointment Fluoride treatment (D1206)
