In addition to your included preventative services, you will enjoy a 15% discount on all other dental treatment. Your annual individual plan cost is $265. You can add an additional individual adult family member of the same household for $235/year, and a child under the age of 14 for $185/year.
All Plans Include:
• Includes a dental cleaning, exam, and fluoride treatment two times per contract period (12 months beginning at your sign up date)
• 1 set of check-up x-rays per year
• a complete set of x-rays eligible every 3 years
The annual membership fee is $265 for the first member of household, $235 for each additional household adult and $185 for each additional household child. Plus a discount on all other dental services that ranges from 15%-25%.
